Duties and Responsibilities
Review Information
Review the information provided on the client plan of care and ask questions of the Staffing Coordinator and/or Nurse Care Manager.
Be Observant
Be observant and report any changes or concerns regarding the client’s condition and the services being provided.
Infection Control
Follow basic infection control practices when providing services to a client.
Personal Hygiene Assistance
Assist the client with personal hygiene as outlined on their client plan of care such as bathing, nail care, oral hygiene, toileting, mobility/transfer, dressing, feeding, etc.
Meal Preparation
Assist the client with meal preparation and clean up as outlined on their client plan of care. This includes preparation and monitoring of meal consumption.
Housekeeping
Complete basic housekeeping tasks as outlined in the Care Plan.
Transportation
Assist with transportation of client to and from appointments, shopping, etc. as directed by the Care Plan.
Documentation
Complete documentation timely and as directed following company policy.
Vital Signs
- If vital signs are required to be taken it will be clearly indicated on the Care Plan
- Equipment will be provided by the Client in the home
- Record the vital signs on the “Vital Signs and Weight” record as well as in your ClearCare or Telephony.
Medication Reminding
- Medication Reminding is when the Client knows which medications they are taking but just needs an auditory or verbal reminder
- The reminder cannot be physical in any manner
- The Care Manager has determined this level is appropriate for the Client and the Client is able to self-direct over their own medication
- This level will be listed on the Care Plan under Medication Manager as Medication Reminder
Emergenices
Life Sustaining Treatment/POLST
- This form is PINK and should be posted on the refrigerator or the wall next to the front door.
- This form outlines the Client’s wishes regarding treatment when they suffer a life threatening event.
- It is the Caregiver’s responsibility to read the POLST and to understand what it means. If you have any questions about the POLST, contact a Nurse Care Manager.
- If the paramedics are called to the Client’s home, Caregivers must direct them to the POLST.
- Keep in mind that not all Clients have a POLST in place.
- If in doubt about your Client’s code status, refer to the Client’s Care Plan or call a Nurse Care Manager.
Medical and Non-Medical Emergencies
- Remain calm and survey the situation
- Assess the Client’s condition and contact 911 if needed
- Refer to Client’s Care Plan and POLST
- Notify New Horizons of the emergency as soon as possible
