Incident Report

Report the incident to appropriate New Horizons administrative staff as soon as possible Complete an incident report whenever you or a Client experiences physical trauma, or when an incident occurs which might result in trauma or injury during your shift Incident reports must be turned in to the office within 24 hours of incident Use this form to document injuries or incidents which have occured prior to shift as well Report all incidents verbally to a Care Manager or Scheduling Coordinator as soon as possible If unsure of whether or not you should complete an Incident Report, contact a Care Manager or Scheduling Coordinator

Medication Incident Report

Complete whenever an unusual event occurs which involves medication Call New Horizons and notify the Care Manager of any incident as soon as possible Must be turned in to the office within 24 hours of the incident

Working/Potential Hazard

Used to identify hazards in your Client’s home All information documented on this form must be verbally reported to a Care Manager as soon as a hazard or potential hazard is identified Must be turned in to office within 24 hours of identifying hazard or potential hazard

Medical Visit Report

You must complete this form prior to any medical visit that you accompany a Client to At the beginning of the visit, the doctor will review the form, make notes, and sign After the visit, you will report findings & changes to the Care Manager Forms should be turned in to the office as soon as possible
