The Job of a Case Manager

What does a case manager do? They wear multiple hats! A case manager is there for assistance for their clients. Together, case managers assist their clients in creating goals, case plans, resource needs, and financial plans. Monitoring progress for the cases and aiming for success for their clients are goals of their own. A goal of the case managers is to assist their clients with their needs to live life to their fullest by connecting them to resources, become an advocate for their client, and a goal-oriented improvement in their case needs.
