A Care Manager will meet you at the hospital and perform a thorough pre-discharge care consultation and care coordination. We will work with you, your doctor and healthcare team to create a personalized care plan based on your specific needs and preferences. We work with the hospital discharge planners to ensure that all of the necessary steps have been taken to prepare for a safe discharge, so all you have to worry about is your recovery.

Getting you home safe and worry-free.

On the day of your discharge we coordinate your transportation home, along with any prescription and grocery pickup needs you may have. We make the discharge process as seamless as possible to ensure your transition home is a successful one.

Recover safely at home.

Hospital to Home is a 30-day program that ensures you have everything you need to recover safely at home.

Once home, one of our Care Managers will meet with you for a comprehensive in-home assessment, including remote patient monitoring set up, home safety evaluation and medication reconciliation.

Following your in-home assessment you will receive daily monitoring and nurse support to make sure you continue to heal safely at home. Before the 30 days are through, you’ll receive a reassessment by your Care Manager to determine your ongoing care needs.
